Clients are referred by the Continuing Care Division of the Vancouver Health Department.

To arrange for a referral/assessment, call an intake worker call at the North Community Health Office, Long Term Care at 604-253-3575 or Evergreen Community Health Office at 604-872-2511. Or you can phone directly to the Lion's Den at 604-718-5848 and we will help you make arrangements. Waitlists as of May 2006 are short.
